Individual education plans & 6th form transition passports
“Children and young people with Complex Learning Difficulties and Disabilities (CLDD) have conditions that co-exist. These conditions overlap and interlock creating a complex profile. The co-occurring and compounding nature of complex learning difficulties requires a personalised learning pathway that recognises children and young people’s unique and changing learning patterns.”
Individual Education Plans (IEPs)
Pupils’ curricula starts with their Individual Education Plan (IEP) and defines what is important to the child/YP and their provision starts with what is important for them. Our curriculum model assumes a responsibility to deliver against pupils’ independence and wellbeing outcomes. A blend of pupils’ personalised learning needs helps to define the core curriculum offer.
6th form transition passports
When pupils move into the 6th form their personalised learning is outlined through a transition passport. The principle function of this is to ensure that the pupil’s learning is finely-tuned towards preparation toward the young person’s next setting.
